Navigate to Account

Select Offices.

Ensure the Facility Code (POS) is set to 12, Home. If changes are needed, select Edit.

Use the drop-down to select 12- Home.

Press Save once complete.

Navigate to Billing > Live Claims Feed.

Enter the patient's appointment by pressing on the Date of Service.

Press HCFA/1500 to display the claim form. The patient's home address will appear in box 32.

The patient's home address will appear in Boxes 5 and 32.

The patient's home address will appear in Box 32: Service Facility Location.
