Steps

Users can view and create custom patient demographics from the patient chart via the demographics tab.

Scroll down to Custom Demographics, and click Manage Custom Patient Demographics.

Users can also view and/or create custom patient demographics by navigating to Account > Custom Fields.

Click Patient Demographics.

Drag to reorder existing custom demographics.

Edit or archive existing custom demographics.

To create a custom demographic, click add new field.

Name your custom demographic. For this example, we will label this field "Referral Source".

Custom Patient Demographics with a Form Name can be inserted into a clinical template via Clinical Form Management.

Field type options
- Text Field- free type.
- Text Area - expanded free type.
- Single/Multiple Select - a dropdown that allows one or more options to be selected.
\*allowed values (for single/multiple select) - enter each value on a separate line by hitting return on your keyboard.

Users can make a custom demographic available and/or required for patients via the check-in app or onpatient.

Cancel or save when finished.
If saved, it will appear in the list of your custom demographics.

Your new custom demographic will appear in the patient chart!
